KINDERGARTEN AND NEW FIRST GRADE REGISTRATION FOR 2012-13
December 2012
Registration for new students entering kindergarten and first grade in the 20012-13 school year will take place on January 26th 2012.
Location: Central Elementary School
Time: 7:30 AM - 11:30 AM
2:00 PM - 5:00 PM
Children entering kindergarten must be five years old on or before October 1, 2012.
Parents must present the following at registration:
1. Original birth certificate and one copy
2. Original proof of residency and one copy
3. Immunization Records from child’s health care provider
4. Confidential Health history form completed by parent (attached)
5. Completed Physical Examination form signed by your child's health care provider and dated on or after
September 1, 2011 (attached)
6. Completed Registration form (attached)
7. Extended Day Kindergarten Program registration form and check (if interested
in child care through HCC)
A completed Physical Examination form and documentation of all mandated immunizations are required for all new kindergarten and first grade students before entering school in September. All medical information must be submitted to the school nurse before your child can attend school in September.
All forms are available in electronic format. You can also pick up hard copies in every School Office, the Superintendent’s Office, or at the Public Library. Parents are urged to obtain the registration packet ahead of time so that it can be completed and presented on registration day.
Information and registration for the Extended Day Kindergarten Program is included in this packet. If you plan to register for EDKP at this time, please be prepared to pay a $50.00 non-refundable registration fee, made payable to HCC. As space in this program is limited, parents are urged to register early for EDKP. You may call Haddonfield Child Care at 429-1603 for further details. Also, we have included a copy of the Haddonfield Board of Education Policy (#2312) regarding Elementary School Class Size and Assignments.
