Course
Request Change
Steps for requesting a schedule change:
1. See your counselor.
2. Fill out "Course
Request Change" form or pick one up in the
Counseling Office.
3. Have your parent/guardian sign form.
4. If dropping course - return materials (book)
and have teacher
sign form.
5. Return the form to your counselor.
**If you drop a full-year
course after the first marking period, you will
receive a 60 (F) as your final grade.**
Opt Out Form for Military Recruiting
Option 2
Form-Option 2 gives students the opportunity to take
approved courses outside of the high school.
Approved courses will appear on student
transcripts, but do not factor into the GPA.
Option 2 may include, but is not limited
to, one or more of the following:
interdisciplinary or theme-based programs:
independent study, magnet programs, student
exchange programs, distance learning,
internships, community service, co-curricular or
extra-curricular programs, and/or other
structured learning experiences.
